Clinical Management Services

  • Providing consistent availability of Nurse Practitioners in the facility
  • Timely and efficient response to resident needs, condition changes, injuries / accidents, etc.

  • Coordination of care with Attending Physicians / Interdisciplinary Team
  • Thorough examination, testing, diagnosis, and treatment of resident conditions

  • Pain management services including

    • Evaluation of resident current and historical pain, initiating and relieving factors, etc

    • Recommendations for implementation and/or modification of medication regimen

    • Implemenation of non-pharmacological, ancillary pain relief methods

  • Falls Management Services including:

    • Assessment of resident history of falling, contributing physical factors, medication use, etc
    • Review the evaluation of environment, incontinence and other potential factors contributing to falls
    • Determine need for assistance devices, hearing and vision testing, osteoporosis screening, etc.
  • Collaboration with interdisciplinary staff to evaluateurinary incontinence, catheter use, infection, timely treatment, etc.

  • Reliable completion of required regulatory evaluation and documentation

Wound Management Services

 
  • Early screening, focused assessments, identification of risk for alterations in skin integrity

  • Routine follow-up visits for evaluation and documentation of healing process

  • Establish wound care protocols to ensure consistent approach to wound management and support of moist wound healing

  • Reliable completion of required regulartory evaluation and documentation, (specific to F314)

  • Physicians and certified Wound Specialist available for consultation

Risk Management Services

  • Critical Incident Technologies and Infinity NP have teamed up to redefine risk management.  Incident Solution provides prompt information for risk management, legal, and corporate administration, while Infinity NP provides proactive incident intervention with Nurse Practitioners. The combination of these two entities provides timely, proactive evaluation and rapid intervention of potential risk management incidents.

     Infinity NP (INP) provides quality outcome and risk management resources and consultation services to facilities and health care groups.  Utilizing industry standards, INP combines assessment and analysis of specific facility and group data with current standards of care to assist in improving risk management and quality outcomes.  Attention to specific facility concerns allows us to individualize approaches in order to improve care outcomes and maintain resident satisfaction.  Our focus on early intervention in clinical issues such as pain management, falls management and pressure ulcer prevention and management, allows us to assist in construction of evidence-based solutions in identified areas.

    Our staff of Nurse Practitioners has a depth of experience in quality assurance, quality improvement, and risk management.   We share our expertise by providing education and data analysis, monitoring, and structured feedback.  Our goal is to help our customers improve incident tracking, trends in care, and quality resident outcomes.  Facility risk is reduced through identification of risk, proactive education, and targeted process modification.  This approach will maintain individual and institutional integrity, promote resident / client well being, and therefore be consistent and compatible with the legal needs of the healthcare facility.
